Serious question, can anyone here explain why Kerala is much cleaner than other South Indian states, given its actually poorer than Karnataka, Telangana and Tamil Nadu. Is it cultural or somehow their municipalities function properly ?

Because Kerala depends a lot on global tourism revenues. You have to keep your cities and towns clean for good paying foreign tourists to visit. Other states only attract a fraction of tourism that Kerala does.
