If this is how it is in a country like the US you can well imagine what's it like in India with massive corruption added to the above mess. Why blame people up north from flocking to appear for such examinations while paying an arm & a leg for coaching classes then ?

Situations happen. What distinguishes the public sector from the private one is that accountability is fixed , the errant penalized whatever form that penalization takes from a red mark on the annual confidential report onwards & the system rectified to prevent repetition otherwise there's public money from the exchequer to bail out the public sector . There's only the IBC for the Pvt sector.in the world of siloed systems and logics, sometimes in an effort to build efficiencies some aberrations slip thru the cracks without anybody's fault. deliberate acts of dishonesty are a separate issue.
in another case in west asia, a company spent a few millions in $ to add fingerprint access system, only to realise later that workmen's fingerprints gets erased while doing manual labour, which made the whole system redundant.
solution is that, have people in the team who have the ability to think out of box, who can catch anomalies before they happen as much as possible. people like me
